Diversity Scholarship Application Form

 

Please use the space below to indicate why you would like to apply for a Diversity Scholarship and how you would contribute to the school. Please include details of any academic projects and challenges that you have already undertaken as well as your aspirations for the future. 

 

Please write no more than 250 words. The writing must be your own work.

Please provide the names and email addresses of two teachers who can complete a recommendation form for you. (Note: We will email the recommendation form directly to the teachers)

Grade 6-12 applicants only:  

Please attach a 1- 2 minute video introducing yourself to the Scholarship Committee. Tell us why you are deserving of a Daystar Academic Diversity Scholarship. Upload the video as an attachment with the Scholarship Application and supporting documentation. 

Please upload the progress report of the last term, video, and supporting portfolio of evidence (e.g. external examination results, awards, certificates, testimonials, published work etc.)
